Home Health Care – Admissions Status

Notice of Service Capabilities and Limitations

Services Offered Sunshine Home Health Care offers the following services: Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Home Health Aide/Bathing, and Medical Nutrition Therapy.

Patients We Serve Sunshine Home Health Care provides care for many different diagnoses and clinical presentations. Our patient population is generally adults, and our referrals come from hospitals, nursing facilities, provider offices, and congregate living facilities.

Visit Frequency Policy Sunshine Home Health Care generally does not provide daily visits for any discipline, with limited exceptions. Our discipline frequencies range based on patient need, but daily needs of any particular discipline are often best met in an inpatient setting.

Exceptions to this are for very short duration and are approved by clinical leadership for specific cases, such as short-term daily nursing to teach caregivers, or daily PT for intensive training or crucial post-op care. When staffed for bathing visits, they are offered a frequency of 1 time/week.

Insurance and Qualifying Providers Sunshine Home Health Care accepts patients who have insurance that we are contracted with to provide care. We also provide care in private-pay situations.

In all cases, patients must be under the care of a qualifying provider who orders and signs for the home health services rendered (e.g., MD, DO, DPM, PA-C, and ARNP) and must meet the home health criteria set forth by their insurance company.
